Conceptual Framework for Understanding Incident Management Systems During Public Health Emergencies

Objective:
Effective incident management is essential for coordinating efforts of multiple disciplines and stakeholders when responding to emergencies, including public health disasters such as the ongoing coronavirus disease 2019 (COVID-19) pandemic.
Methods:Existing research frameworks tend to focus on formal structures and doctrine (eg, ICS-NIMS); however, organizational processes that underlie incident management have not been systematically assessed and synthesized into a coherent conceptual framework.
Results:The lack of a framework has hindered the development of measures of performance that could be used to further develop the evidence base and facilitate process improvement. To address this gap, we present a conceptual framework of incident management drawn from expert feedback and a review of literature on incident management and related fields. The framework features 23 measurement constructs grouped into 5 domains: (1) situational awareness and information sharing, (2) incident action and implementation planning, (3) resource management and mobilization, (4) coordination and collaboration, and (5) feedback and continuous quality improvement.
Conclusions:As such, the article provides a first step toward the development of robust measures for assessing the performance and effectiveness of incident management systems.

Background
Studying phenotypic and genetic characteristics of age at onset (AAO) and polarity at onset (PAO) in bipolar disorder can provide new insights into disease pathology and facilitate the development of screening tools.
AimsTo examine the genetic architecture of AAO and PAO and their association with bipolar disorder disease characteristics.
MethodGenome-wide association studies (GWASs) and polygenic score (PGS) analyses of AAO (n = 12 977) and PAO (n = 6773) were conducted in patients with bipolar disorder from 34 cohorts and a replication sample (n = 2237). The association of onset with disease characteristics was investigated in two of these cohorts.
ResultsEarlier AAO was associated with a higher probability of psychotic symptoms, suicidality, lower educational attainment, not living together and fewer episodes. Depressive onset correlated with suicidality and manic onset correlated with delusions and manic episodes. Systematic differences in AAO between cohorts and continents of origin were observed. This was also reflected in single-nucleotide variant-based heritability estimates, with higher heritabilities for stricter onset definitions. Increased PGS for autism spectrum disorder (β = −0.34 years, s.e. = 0.08), major depression (β = −0.34 years, s.e. = 0.08), schizophrenia (β = −0.39 years, s.e. = 0.08), and educational attainment (β = −0.31 years, s.e. = 0.08) were associated with an earlier AAO. The AAO GWAS identified one significant locus, but this finding did not replicate. Neither GWAS nor PGS analyses yielded significant associations with PAO.
ConclusionsAAO and PAO are associated with indicators of bipolar disorder severity. Individuals with an earlier onset show an increased polygenic liability for a broad spectrum of psychiatric traits. Systematic differences in AAO across cohorts, continents and phenotype definitions introduce significant heterogeneity, affecting analyses.

The present study investigated the effects of different types of recasts and prompts on the rate of repair and spontaneous use of novel vocabulary by eight children with severe motor speech disabilities who used speech-generating technologies to communicate. Data came from 60 transcripts of clinical sessions that were part of a conversation-based intervention designed to teach them pronouns, verbs, and verb inflections. The results showed that, when presented alone, interrogative choice and declarative recasts led to the highest rates of child repair. The results also showed that when children were presented with recasts and prompts to repair, the rate of repair increased. Spontaneous use of linguistic targets was significantly and positively related to conversational sequences where the adult recast was followed by child repair. These findings suggest that using different recast types and prompts to repair may be beneficial for spontaneous use of linguistic targets in this population.

Summary
INTRODUCTION
The surface area of Saturn's rings is greater than that of any of the planets in the solar system, yet, aside from dust, we have never observed or sampled an individual ring particle. Rings are unique in the solar system in that they are a complex dynamical system whose individual constituents interact not only with the light that we use to sense them remotely, but also with each other through gravitational and contact forces. These dynamical interactions play as large a role in determining the appearance of the ring system as do the optical properties of the individual ring particles. In this chapter we review the experimental work that has been done to help us understand both aspects of planetary rings: their collective dynamical behavior and their optical properties.
We have a wealth of data on the behavior of ensembles of particles, both dynamically and their optical properties. Laboratory measurements of the behavior of various likely ring particle analogs are a critical link in connecting these bulk observations with the nature of individual ring particles, and understanding the properties of individual ring particles should provide clues to the outstanding unanswered questions about the age and origin of rings.
Images of Saturn's rings and optical depth profiles from occultations show features at a variety of spatial scales, from the resolution limit of tens of meters for occultations up to thousands of kilometers, and including most scales in between (Colwell et al., 2009; Chapter 3). A frustratingly small fraction of these structures is well understood. Many that remain puzzling, such as the large optical depth fluctuations in Saturn's central B ring, the complex structure in the B ring and the inner A ring, the long-wavelength low-amplitude undulations in optical depth in the C ring, and the plateaus in the C ring, are likely linked to either the collective behavior of the ring particles governed in part by their collisional properties (see e.g. Schmidt et al., 2009, for a review) or by ballistic transport of material due to extrinsic micrometeoroid bombardment (Chapter 9). The mechanical properties of individual ring particles are critical in both types of process.

The current gold-standard therapeutic strategies for bone grafts in the patient population are to use either allograft or autograft bone. Although these approaches have a long track record of utilization, neither is without risk to the patient, and there remains a desire in the field to improve treatment options. While there have been treatments approved by the FDA for full length growth factors and calcium salt-laden collagen sponges, these are not available for the entire population of potential bone graft patients. One viable strategy to focus on these concerns is to design an implantable bone graft substitute that can address all the negative drawbacks of autograft bone, allograft bone, and full length proteins. The work provides a preliminary investigation of synthetic, nanofiber-permeated, composite polymer/ceramic scaffold for bone repair using thermally induced phase separation, PLLA microspheres, and hydroxyapatite. The scaffolds as described have fiber diameters that mimic natural collagen ECM networks in bone as determined by scanning electron microscopy and will serve as the basis for future studies in substrate-guided bone tissue regeneration.

The challenge in summing up the active life and productive career of any famous person is to put the important facts on record while steering a respectful path between hagiography and recitation of his curriculum vitae. We attempt to do this for our esteemed colleague and mentor, one of Mesoamerica's last originals, Gareth W. Lowe (Figure 1). With his death on March 8, 2004, in Tucson, Arizona, at age 82, Gareth left behind a legacy that reaches far beyond his well-deserved international reputation. Gareth spent 50 years of his life working in Mesoamerica, principally on the Formative period in Chiapas, Mexico. He first went to southern Mexico in 1953 as a crew member of the newly organized New World Archaeological Foundation (NWAF). Two years later, the NWAF was rescued by the sponsorship of the Church of Jesus Christ of Latter-Day Saints, and much later (in 1976) it became part of Brigham Young University. Gareth served two stints as NWAF field director (1956–1959, 1961–1975), basically as de facto director, and finally served officially as director from 1975 to 1987. In a real sense, Gareth was the NWAF, and its successes and agenda were largely his. The story of one cannot be understood without the other.

The Bahamas is a group of islands, just off the coast of Florida, extending southwards to near the island of Hispaniola. There are approximately 25 inhabited islands and numerous small islands and cays that are uninhabited. The population of the Bahamas is approximately 230–240,000. A census taken in 1980 showed that approximately 65% of the population is aged between 15–45 years. The major industries lie in tourism and off-shore banking. The Bahamas has not always had a drug problem; the diagnostic records at the mental hospital, Sandilands Rehabilitation Centre, reveal that in the sixties drug-related admissions were negligible. In the early seventies, however, there was a rapid increase in cannabis use and at the same time the number of drug-related admissions increased. The patients were mainly admitted in acutely disturbed states which were thought to be drug psychoses caused by cannabis inhalation. There do not seem to have been many patients requesting help to stop using cannabis, and generally there has been no significant change in this pattern. The major difference is the vast increase in the quantities smoked by an individual user. In the mid-seventies, the pattern of drug use began to change in that more patients were using a number of drugs at the same time. Methaqualone enjoyed a period of popularity and was known on the street as Quaaludes, Mx or Disco Biscuit. The drug's ability to potentiate the action of alcohol made it both dangerous and alluring, as it was possible to get ‘stoned’ almost immediately for the price of only one drink and one pill. It was about the same time in the mid-seventies that cocaine hydrochloride began to arrive on the market in increasing quantities and start gaining in popularity. The major problems at that time were associated with drug-induced psychoses, acute intoxications and occasions of violent bizarre behaviour due to the disinhibition and release of aggression caused by these drugs. One drug addict from this era, interviewed in the prison, said that a good night out was to smoke some grass, drink a Guinness stout, take a quaalude and then snort some cocaine. There is really no literature as to what effect these combinations may have on the brain. It is of interest however that very few patients were presenting for treatment of addiction, though by the late seventies a trickle had begun to come forward. Cocaine was not that plentiful and was being used by snorting, i.e. the inhalation of the finely ground white powder into the nostrils. A less popular method of use of cocaine at that time was smoking the finely ground powder as a constituent of a tobacco cigarette or mixed into the contents of a marijuana cigarette. The quantities of cocaine available on the Bahamian street market at that time were small.
Estimation of the proportion of non-ammonia-nitrogen reaching the lower gut of the ruminant derived from bacterial and protozoal nitrogen

1. A method for estimating the proportions of bacterial- and protozoal-N in the total non-ammonia-N reaching the lower gut of the ruminant under steady-state conditions was evaluated. Three trials using two different diets were conducted with a Holstein steer equipped with a rumen cannula and duodenal re-entrant cannulas.
2. An intraruminal primed infusion of (15nh4)2so4 was administered for 68 h during each trial. Bacteria and protozoa samples were isolated from rumen fluid at approximately 6 h intervals during each infusion period. Total non-ammonia-N was isolated from duodenal digesta samples taken at approximately the same times. All of these samples were analysed for 15N enrichment. A computer program was used to fit equations to the 15N-enrichment curves of bacterial- and protozoal-N. Models of both bacterial- and protozoal-N kinetics consisted of a small pool which equilibrated rapidly with rumen NH3 and a large pool with a fractional turnover rate of 0.045–0.070/h for bacterial-N and 0.056–0.069/h for protozoal-N.
3. Abomasal fluid turnover was estimated by a single injection of polyethylene glycol (molecular weight 4000) into the rumen followed by sampling of rumen fluid and duodenal digesta.
4. Estimates of abomasal fluid turnover, bacterial-N turnover, and protozoal-N turnover were entered into an equation which was adjusted by computer iteration to fit the 15n-enrichment curve of duodenal digesta non-NH3-N generated from each (15nh4)2so4 infusion period. The computer fit of this equation to the observed results gave estimates of 0.39–0.45 and 0.22–0.41 for the proportion of duodenal non-NH3-N derived from bacterial-N and protozoal-N respectively.
5. This method is potentially useful in estimating microbial protein passage to the lower gut in ruminants. Sampling digesta from the omasum rather than the duodenum would simplify the method and possibly increase the reliability of the estimates.
